HMO plans typically require members to select a primary care physician (PCP) who acts as the main point of contact for all healthcare needs. PCPs coordinate and manage referrals to specialists within the HMO network. HMO plans generally offer lower out-of-pocket costs but have more restrictions on seeing out-of-network providers.

EPO plans are a hybrid between HMO and PPO plans. They have a network of preferred providers, and members must seek care within the network to receive coverage. EPO plans generally do not require referrals for specialist visits, but like HMOs, they usually do not cover out-of-network care, except for emergencies.

PPO plans offer more flexibility in choosing healthcare providers. Members can see any provider within the PPO network without a referral, and they can also visit out-of-network providers, albeit with higher out-of-pocket costs. PPO plans usually have higher premiums compared to HMO plans but provide more freedom in provider choice.

POS plans combine features of HMO and PPO plans. Members select a primary care physician within the network who manages their healthcare needs and provides referrals to specialists. POS plans allow members to see out-of-network providers, but at higher costs. They offer more flexibility compared to HMOs but usually have higher premiums.
